Email address incorrectly filling into date fields

If you use the LastPass browser extension to store your Forms Live login credentials, you may experience an issue with incorrectly filling fields.

To rectify, please disable the browser extension by following one of these option:

Option 1

1. Click the red icon next to the filled field

2. Click More options

   

3. Click Turn off LastPass for this site

   

4. Click Turn off for this site

   

Option 2

1. Click the LastPass icon in the top right corner of your browser
2. Click Vault

3. Click Advanced Options in the left menu

4. Click Autofill settings

   

5. Click the red Add new button

6. Enter the site URL for your state from the below list

https://sa.formslive.com.au/

https://vic.formslive.com.au/

https://tas.formslive.com.au/

7. Choose Never do anything from the Action list

8. Click Save

9. Head back to Forms Live and refresh the page

NB: this may take a few minutes for LastPass to register the change. You may also need to close and reopen your web browser.
