Signee Verification

Make sure that the correct mobile number is entered when filling out the form. If an incorrect number was used, see the Edit & Resend section.

All signing parties will be notified by email that their signature has been requested.

  1. Click the View & Sign Form button within the email.

  1. To verify your identity, enter your mobile number – no spaces – then click Verify Mobile

  1. A One Time Password (OTP) will be sent via SMS from Forms Live

  2. Enter the 6-digit number in the OTP Code field.

  3. Click the Submit & Verify button.

  1. You can now view the form.
